AWARD NOMINATION FORM

This is the official Southwest Chamber of Commerce Nomination Form for the 2007 A·C·E Awards. You can use the form below to nominate any Chamber member including your own business/non-profit organization. While you may nominate as many businesses or organizations as you like, please, nominate each business/non-profit organization for one award category only.

All nominations are due by June 1, 2007.

PURPOSE

The A·C·E Awards are designed to recognize the efforts, commitments and achievements of businesses and non-profit organizations that are members of the Southwest King County Chamber of Commerce who help make the Burien, Des Moines, SeaTac and Tukwila communities a viable place to work and reside.

ELIGIBILITY

Any business or non-profit organization that is a member-in-good standing of the Southwest King County Chamber is eligible.

CATEGORIES

LARGE EMPLOYER BUSINESS EXCELLENCE
A business with more than ten employees that has achieved excellence through innovative business practices, products, and/or employee programs and community commitment.
Judging Criteria: Customer Service 40% Leadership/Innovation 30% Community Involvement 30%

SMALL EMPLOYER BUSINESS EXCELLENCE
A business with ten or less employees that demonstrates the same qualities listed above. Judging criteria will be the same as above.

NON-PROFIT/HUMANITARIAN SERVICE
A non-profit organization that distinguishes itself through meeting a defined public need, providing outstanding service to its constituents and community involvement.
Judging Criteria: Impact on Constituents 60% Innovation of Services 40%

BEAUTIFICATION
A business or organization that has enhanced the attractiveness of the community by constructing a new building, remodeling its exterior or interior, or significantly improving its landscaping.
Judging Criteria: External 60% Internal 40%

PEOPLE’S CHOICE AWARD
An outstanding business or organization (from any industry) that exemplifies quality management, customer service, and community spirit . . . selected by our membership (write your People’s Choice Award Nominee below).

JUDGING

Once the Nomination Forms are received in the Chamber Office, staff will compile the results and the top five companies in each category will be part of the vote, by Chamber Members, to elect the winners. Voting will take place in August 2007.
